This Loyalty Program is not an insurance plan, but a payment arrangement provided by Dr. Luciana Bretz-Pavie DDS., and is not transferable to any other dental office or dental specialist
Payment for services is due at the date of service
This program is for you and is not transferable
Benefits are provided for one year from the date the program is purchased
Must be paid in full when starting the program
No refunds will be provided for dues of Program Annual Cost under any circumstances, including failure to schedule and maintain appointments
To receive the 10% discount on restorative treatment, you must schedule the appointment the day it is recommended. Any failed appointment results in the loss of the 10% discount.
